Important Note

The transfusion reaction workup must be initiated by the nursing unit per the physician's order. Initial signs and symptoms of a hemolytic transfusion reaction are: Fever >1.0°C, Hemoglobinuria, Chills, Shock, Chest pain, Generalized bleeding, Hypotension, Oliguria or anuria Nausea, Back pain, Flushing Pain at infusion site, Dyspnea

1) Stop transfusion immediately and keep IV open with saline. 2) Notify lab and physician immediately. 3) Check for agreement between blood bank record, component tag, and patient arm band. 4) Obtain urine specimen 5) Complete Transfusion Reaction Report form and send to lab with remainder of blood bag.
