Important Note

One Level Dosing: In most cases, a single vancomycin level (MIDPOINT) will be ordered. It will be drawn halfway through the dosing interval.

Pharmacists will time these levels when ordering: 12 hours after the dose for q24h dosing, 6 hours after the dose for q12h dosing, 4 hours after the dose for q8h dosing

Specimen Requirements

Preferred Collection Container: green top lithium heparin tube, SST 

Also Acceptable Collection Container: serum red top tube or EDTA purple top tube

Collection Information:

Collect 1 mL of Lithium heparinized plasma or serum.

Serum or plasma must be separated from cells within 2 hours of collection.
